An AI Agent Destroyed a Production Database. The Agent Wasn't the Problem.
Fortune reported this week that an engineer let Claude Code run end-to-end on a production system without permission gates on destructive operations. The agent wiped the database. Separately, Amazon internal docs reveal a pattern of AI-caused outages — and the company initially blamed "Gen-AI assisted changes" before scrubbing that language ahead of a leadership meeting.

The engineer's mistake wasn't using an AI agent. It was a configuration choice: letting the agent run write operations against production without explicit human approval or a pre-approved allowlist. That's not an AI problem. That's an ops problem.
We run agents in production daily. The rule is simple: any write operation against prod requires either explicit human approval or a pre-approved allowlist. The agent can read whatever it needs, analyze whatever it wants, and draft whatever changes make sense. But the commit to production goes through a gate. Every time.
The Amazon story is more interesting. Their internal docs initially blamed "Gen-AI assisted changes" for a trend of incidents, then they scrubbed that language before the meeting. That's the kind of organizational denial that causes the next outage. If you can't name the problem accurately inside your own company, you can't fix it.
The pattern across both stories is the same: agents given production access without constraints, operating in environments where no one built the safety layer. The agent did exactly what it was told. The environment failed to protect itself.
Safe agent deployment isn't complicated. It requires permission boundaries, approval flows, and the willingness to call failures what they are. Companies that build these constraints into their agent workflows from day one don't end up in Fortune articles.
